• Job Description: Speech Language Pathologist in Home Health

• We are seeking a highly skilled and compassionate Speech Language Pathologist to join our team in providing exceptional care to our patients in their homes. As a Speech Language Pathologist in Home Health, you will be responsible for assessing, diagnosing, and treating patients with communication and swallowing disorders.

Responsibilities

• - Conduct comprehensive assessments of patients' communication and swallowing abilities
• - Develop individualized treatment plans based on assessment results
• - Provide therapy to patients to improve their communication and swallowing abilities
• - Educate patients and their families on communication and swallowing strategies
• - Collaborate with other healthcare professionals to ensure coordinated care for patients
• - Maintain accurate and timely documentation of patient progress and treatment plans

• Requirements

Requirements

• - Master's degree in Speech Language Pathology from an accredited program
• - Current state license to practice as a Speech Language Pathologist
• - Certificate of Clinical Competence (CCC) from the American Speech-Language-Hearing Association (ASHA)
• - Minimum of 1 year of experience in a home health setting
• - Strong communication and interpersonal skills
• - Ability to work independently and as part of a team
• - Valid driver's license and reliable transportation
