  • The Center for Recovery and Wellness takes a holistic, strengths-based approach to recovery and wellness that responds to the distinct needs of each individual.  Programs include residential and outpatient addiction treatment, recovery services, prevention education, and community programming.  CRW offers culturally competent, evidence-based programs and services that strengthen individuals and communities, enhance resiliency, and foster lasting recovery. 
  • We are seeking a full-time Reintegration Coordinator to counsel and aid individuals who have substance use disorders through case management in Pride Site, a New York State OASAS-certified residential treatment program. Utilizing a person centered and trauma informed lens; individualized assessment and counseling services are delivered in Pride Site.  Case management strategies are applied throughout the individual’s transitional stage of re-entry to the community. During this stage individuals live in the residential environment and receive outpatient services at the Center for Recovery and Wellness.

  • RESPONSIBILITIES
  • * Provide up to 20 clients with biweekly individual case management sessions in collaboration with the vocational services team to ensure adherence to vocational and treatment schedules, and a smooth transition to the community. 
  • * Coordinate services with vocational, psychiatric, medical, mental health, and other social service providers.
  • *  Communicate with on-site outpatient program to ensure clients on caseload are receiving and responding to clinical care in the outpatient program while residing in the treatment facility and adhering to residential program reintegration expectations.
  • * Organize and participate in multidisciplinary interventions for clients on caseload.
  • * Provide follow-up interventions, assessing progress, and communicating results to interdisciplinary team.
  • * Provide evidence-based group counseling to clients in the residential program and coverage in the outpatient program.
  • * Participate and provide detailed progress reports in case conference for both Pride and PCO programs 
  • * Complete detailed progress notes, comprehensive treatment plans, psychosocial assessments, discharge plans, and other required documentation in accordance with standards.
  • * Engage client and treatment team in warm hand-offs for the step down of care.  
  • * Maintain accurate and organized clinical charts, engaging in pre-audits of charts to ensure compliance with OASAS and managed care standards.
  • * Send weekly, monthly, and quarterly reports to HRA through the Substance Abuse Tracking and Reporting System.
  • * Check toxicology reporting portal daily to ensure any positive toxicology reports are communicated to supervisory staff, documented, and addressed with resident.
  • * Ensure that clients’ annual medical examinations and psychiatric evaluations are completed, completing referrals as needed.
  • * Contact mandating source upon admission of new clients in a timely manner and provide written/oral clinical updates as needed.
  • * Make collateral contacts for and with clients.
  • * Outreach to referral sources to expand network of clients.
  • * Attend staff meetings and trainings as required, as well as individual and group supervision.
  • * Perform special projects and other duties as assigned.

  • QUALIFICATIONS
  • * Master’ Degree (MSW; MHC; etc.), and either licensed in the state of New York or working under a limited permit and eligible for licensure, or CASAC with 3-years of residential experience required
  • * Experience working directly with clients diagnosed with substance use disorder 
  • * Ability to multitask and handle challenging situations in a professional and courteous manner
  • * Maintain confidentiality of records and information
  • * Willingness to work as part of a team and excellent collaborative skills.
  • * Must be organized, self-motivated, detail oriented, and technologically savvy.
  • * Available for shifts to accommodate a caseload of clientele who may be out of the house responding to work/training activities during the standard 9-5 M-F schedule.
  • * Ability to work with a diverse set of clients with differing presentations.
  • * Ability to work professionally and empathetically with a diverse group of clients and staff.
