  • At Independent Living, Inc., as a Recovery and Resource Navigator, you’ll be a welcoming guide for individuals seeking support. You’ll listen closely, help identify needs, and connect people with services that promote their health, stability, and recovery.

  • What You’ll Do
  • Provide initial screenings to understand each person’s needs and help determine eligibility for services or benefits.
  • Use a standardized screening tool to identify social and health-related needs for Medicaid-eligible individuals.
  • Connect clients with appropriate community programs, recovery supports, and social services.
  • Follow up to ensure clients are able to access services and receive the support they need.
  • Monitor email and the UniteUs platform for incoming referrals.
  • Accept, document, and complete outreach for referrals to the SOR program and recovery center.
  • Work closely with SOR (State Opioid Response) Peer Specialists to help clients access care such as detox, residential treatment, outpatient services, or medication-assisted treatment.
  • Coordinate with community partners to help clients overcome barriers like transportation, insurance issues, or documentation needs.
  • Accurately document outreach, screenings, and referrals in SCN systems (including Foothold).
  • Partner with the Program Manager to update workflows, enhance processes, and improve the SCN’s overall impact.
  • Prepare participant success stories that highlight program outcomes.
  • Complete all required reporting and recordkeeping.
  • Participate actively in team meetings, trainings, and professional development opportunities.
  • Support additional tasks as needed to help the program and team thrive.
  • Requirements
  • This Role Could Be a Great Fit If You
  • Willingness to complete required HVCC screening and navigation training within the first week of employment.
  • Experience connecting individuals with disability-related or community services is a plus.
  • Knowledge of local, statewide, or national disability issues and available resources.
  • Strong written and verbal communication skills.
  • Good time management and the ability to stay self-directed, when needed.
  • Valid driver’s license and reliable transportation.
