- The Quality Improvement (QI) Coordinator supervises Quality Improvement staff and is responsible for providing direct support to the center’s Quality Assurance and Improvement Program; including the design and oversight of activities that support CFHC’s key clinical quality improvement initiatives. The QI Coordinator will be focused on projects supporting the implementation and improvement of a robust Patient Centered Medical Home, using team-based care to achieve improved health outcomes as well as improving patient experience. The QI Coordinator will use clinical applications software to design interventions, support teams in carrying out the interventions, and in capturing, analyzing and evaluating data to support workflows and measure improvement.
- Wage Range: $69,305.60- $74,590.36 Annually
- Schedule: Monday Through Friday 8:00 am - 5:00 pm
- FTE: 1.0 - Salary / Non-Exempt

- Requirements
- Bachelor’s degree in healthcare or related field, or equivalent. RN preferred.
- Three years’ experience in healthcare or equivalent, including experience implementing quality improvement initiatives.
- Project management experience with application of QA/QM/QI concepts.
- Knowledge of CLIA, (health care related) OSHA, and HIPAA regulations.
- Knowledge of evidence-based preventive health, chronic disease and primary care.
- Solid background in patient education and lifestyle risk reduction.
- System orientation and experience improving systems functioning and outcomes.
- Experience using and managing electronic health records systems; analyzing and using data from EHR system to improve quality; optimizing use of EHR.
- Risk management experience in a health care setting.
- Knowledge of managed health care systems.
- Customer service experience and strong customer service skills.
- Must be able to tolerate frequent work interruptions, organize work and reset priorities in order to complete work responsibilities in a timely manner.
- Ability to work with a low-income, multi-ethnic patient population and a demonstrated ability to relate respectfully and effectively to people of diverse racial, educational and socio-economic backgrounds.
- Must be a confident, self-directed individual who can work within a team and independently.
- Strong communication, organization, documentation and record keeping skills necessary.
- Valid, unrestricted driver’s license.
