Summary The Pulmonary Program Coordinator will focus on conducting remote monitoring and management for pulmonary/sleep patients. Engage in regular interactions and communication with patients and/or caregivers. Provide health teaching and education on disease processes to patients. This position will be a Respiratory Therapist who will serve as the patient navigator for pulmonary/sleep patients. Job Duties Focuses on conducting remote monitoring and management of pulmonary/sleep patients; work collaboratively with clinical staff, clinicians, and other qualified healthcare professionals; notifies clinician of changes in patient condition or status, obtains new orders, modifies the care plan as needed. Establish and maintain database of patients to be followed; collaborate with IT to create dashboard. Monitor for early warning signs of worsening clinical status or non-adherence to treatment regimen. Engage in regular interactions/communications with patients/caregivers during the month via office visit, virtual visit, and/or phone call. Provide patients with health teaching when clinically indicated, promote patient self-management, and provide education on disease processes. Identify issues related to the healthcare system, transportation, financial, and/or psychological barriers; and work collaboratively to problem solve, coordinate services, and advocate for patients and/or families. Provide care management for the highest risk patients to reduce risk and unnecessary utilization to improve outcomes. Minimum Qualifications Associate’s Degree Graduate of a Registered Respiratory Therapy program accredited by the commission of accreditation for Respiratory Care 3 years experience in one of the following settings: hospital, DME or physician office setting. and Demonstrates knowledge of and complies with applicable laws, regulations, guidelines, and standards as set forth by Regulatory agencies and the American Academy of Sleep Medicine. Strong verbal and communication skills in English. In-depth understanding of disease processes related to CPAP/NIV utilization, including but not limited to PAP adherence, PAP pressure adjustments, mask fittings and hospital to home sleep apnea care management. Time management and prioritization skills. Assessment and critical thinking skills. Clinical experience in with DME, sleep lab testing and inpatient settings is preferred. Knowledge of billing and responsibility for capturing appropriate billing charges under the remote monitoring program. Ability to incorporate strategies for interacting with persons from diverse backgrounds. Effective behavioral and educational strategies such as motivational interviewing, teach-back method, and self- management support. Ability to set priorities to coordinate care plan efficiently. RRT - Registered Respiratory Therapist - National Board for Respiratory Care Upon Hire or CRT - Certified Respiratory Therapist - National Board for Respiratory Care Upon Hire Preferred Qualifications Bachelor’s Degree Certified Professional in Healthcare Quality - National Association for Healthcare Quality Physical Demands Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
