As an Outreach Specialist, you’ll play a vital role in expanding access to care for individuals in need of detox, residential treatment, or observation services by serving as a responsive, knowledgeable, and compassionate connection point for our referral partners across Arizona.
  • This role is ideal for someone with behavioral health experience who thrives in a fast-paced, mobile environment. You’ll serve as the face of Axiom Care to hospitals, case managers, justice-involved systems, and outpatient providers—helping people move into care quickly and smoothly. The position is based in Phoenix, AZ and primarily field-based with a focus on Axiom Care’s programs in partnership with the Navajo Nation, and requires evening, weekend, and flexible availability to support community needs.
  • Yideeska´adi Hózho´o´jí Center (YHC) is a Navajo Nation-owned residential substance use treatment facility in Phoenix dedicated to serving members of the Navajo Nation with culturally appropriate and effective whole-person care. https://navajo-yhc.com/
  • YHC is operated by Axiom Care in partnership with the Navajo Department of Health (NDOH), the Navajo Division of Behavioral and Mental Health Services (DBMHS).
  • Duties/Responsibilities
  • Referral Coordination & Relationship Building
  • Act as a point of contact for incoming referrals and placement coordination
  • Maintain ongoing communication with internal teams to support smooth transitions into care
  • Respond to case manager, provider, and referral source requests with professionalism and urgency
  • Represent Axiom Care’s full continuum of services, including Detox, RTC, Observation, and select specialty programs (YHC, Veteran Tracks, etc.)
  • Outreach & Community Engagement
  • Conduct in-person outreach at hospitals, behavioral health providers, and community sites
  • Host and attend in-services, facility tours, and 1:1 meetings to build strong partnerships
  • Participate in events, resource fairs, and community forums (including evenings and weekends)
  • Provide drop-ins and follow-ups to maintain visibility and trust with key referral partners
  • Communication & Documentation
  • Use platforms like Teams, KIPU, and email to track referrals and communicate updates
  • Maintain timely and accurate documentation of outreach efforts and case coordination
  • Support cross-departmental collaboration by sharing feedback and supporting outreach strategy

  • Requirements

Required Skills/Abilities

  • Strong communication skills, both written and verbal
  • Experience presenting to professional audiences (in-services, case management teams, etc.)
  • Knowledge of Arizona Medicaid/AHCCCS insurance and behavioral health systems
  • Professional presence and comfort working within hospital or provider settings
  • Organized, self-directed, and comfortable managing a field-based schedule
  • Proficiency in Microsoft Office and familiarity with CRM or EMR platforms

Education and Experience

  • High school diploma or equivalent required.
  • Bachelor’s degree or equivalent experience preferred
  • 3–5 years of experience in outreach, admissions, or behavioral health roles required
  • Familiarity with justice-involved populations or hospital discharge planning strongly preferred
  • Personal or professional experience with substance use/mental health treatment preferred.
  • Knowledge of Navajo culture, Navajo treatment system/providers, Navajo language, etc a huge plus.

Work Environment

  • Home office based with frequent travel across the Valley and beyond
  • Reliable transportation and flexible schedule required. Axiom care reimburses for mileage at the IRS rate.
