Job Summary

  • Provides direct and indirect patient care services that meet the psychosocial, physical, and general aspects of care; meets the communication needs of patient and family; provides care that reflects initiative and responsibility indicative of professional expectations, under the supervision of a Registered Nurse. Maintains regulatory agency requirements, nursing and hospital policies, procedures and standards. Communicates with physicians and team members about changes in patient’s clinical condition, including results of diagnostic studies and symptomatology. Is able to respond quickly and accurately to changes in condition or response to treatment. Additionally, is able to perform general nursing duties in all departments with adequate supervision.

Job Responsibilities

  • Demonstrates the knowledge of cardiac monitoring.
  • Follows the five medication rights which reduces the potential for medication errors.
  • Performs patient care responsibilities considering needs specific to the standard of care for patient’s age.
  • Demonstrates ability, under direct supervision of an RN, to gather pertinent information and report to RN on all patients as per policy. This includes: pediatric, geriatric and the general population.
  • Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/heath care errors.
  • Assists with formulation of a teaching plan based on identified patient learning needs and evaluates effectiveness of learning; family is included in teaching as appropriate.
  • Demonstrates ability to perform treatments and provide services to level of licensure.
  • Communicates appropriately and clearly to team leader, co-workers and physicians.
  • Consults other departments, as appropriate, to provide for an interdisciplinary approach to the patient’s needs.
  • Provides care appropriate to condition and age of the patient, including pediatric, geriatric and general population.
  • Demonstrates an ability to assist physicians with procedures and performs services requiring technical and manual skills under the direction of the RN according to scope of practice.
  • Demonstrates an ability to be flexible, organized and function under stressful situations.
  • Treats patients and their family with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families.
  • Demonstrates knowledge of the principles of growth and development over the life span and the skills necessary to provide age appropriate care to the patient population served. Able to gather data about the patient’s status in order to assist with each patient’s age specific needs and provide care needed by the patient group.
  • Interacts professionally with patient/family and staff members.
  • Maintains a good working relationship both within the department and with other departments.
  • Meets current documentation standards and policies.
  • Identifies and addresses psychosocial needs of patients and family; communicates with Social Services/Discharge Planner regarding both in-hospital and post discharge needs.
  • Manages and operates equipment safely and correctly.
  • Coordinates and supervises patient care under the direct supervision of a Registered Nurse.
  • Medical and surgical asepsis is carried out during treatments and special procedures.
  • Performs other duties as assigned within the LPN/LPN II scope of practice.
  • Utilizes Chart Access to compute documentation of patient care activities.
  • May perform duties within the scope of practice according to the rules and regulations of the Board of Nursing in the state of licensure.
  • Performs other related duties as assigned by department supervisor or administration.
  • Requirements

Education/Licensure Requirement

  • Graduate of an accredited LPN program.
  • Current licensure in state of practice as LPN if required by position.
  • Current BLS (Healthcare Provider) certification

Physical Requirements

  • Good physical and mental health and stamina.
  • Lifting and moving moderate to heavy objects.
  • Tolerates prolonged periods of walking and/or standing.
  • Work has occasional stress due to workload pressures.
  • Exposure to blood and body fluids; hazardous materials and waste; airborne disease.
  • Occasional repetitive exposure to unpleasant and/or disagreeable working conditions; (i.e. odors, dirt, grease/oil, noise, fluctuation in temperatures).
  • Basic dexterity-basic eye/hand coordination.
  • Ability to function properly under pressure and stress and cope with emergencies.

Experience Requirements

  • Minimum six (6) months work experience in acute care or nursing home preferred.
  • Basic computer knowledge.

Professional Requirements

  • Adheres to dress code, appearance is neat and clean.
  • Wears identification while on duty.
  • Completes annual education requirements.
  • Maintains regulatory requirements.
  • Maintains patient confidentiality at all times.
  • Reports to work on time and as scheduled, completes work within designated time.
  • Utilizes computerized punch time system correctly.
  • Completes in-services and returns in a timely fashion.
  • Attends annual review and performs departmental in-services, as scheduled.
  • Attends at least 10 staff meetings annually, reads and signs all monthly staff minutes.
  • Represents the organization in a positive and professional manner.
  • Actively participates in performance improvement and continuous quality improvement (CQI).
  • Complies with all organizational policies regarding ethical business practices.
  • Communicates the mission, ethics, and goals of the hospital, as well as the focus statement of the department.
