  • Jet Health, Inc. is seeking a full-time Intake Coordinator to join our team at our corporate office in Fort Worth, TX.
  • The Intake Coordinator is responsible for the timely coordination of the referral process, including the review, response, and coordination of care for patients referred to Jet Health, Inc. This role requires strong communication and organizational skills, as well as specific experience verifying eligibility and benefits for home health services.
  • Requirements
  • Coordinate the referral process for patients referred to Jet Health, Inc.
  • Monitor, review, and respond promptly to patient referrals.
  • Verify and document insurance eligibility and benefits for home health services.
  • Develop and maintain referral relationships within the community.
  • Interact with facility representatives, discharge planners, and other agencies to facilitate care initiation.
  • Conduct reverse marketing efforts in coordination with the Business Development Leader.

Qualifications

  • Education: High School graduate or equivalent, with preference for graduates of accredited vocational/practical nursing schools.
  • Licensure: Current state license as a Licensed Vocational/Practical Nurse or a multi-state license under the National Nurse Licensure Compact.
  • Skills: Compassionate nature, ability to work independently, good communication, reliability, and organizational skills.

Experience & Skills

  • Prior experience verifying insurance eligibility and benefits for home health and/or hospice services is required.
  • Compassionate nature with the ability to work independently.
  • Strong communication, reliability, and organizational skills.
  • Proficiency in using Homecare Homebase or similar EMR systems.

Essential Functions

  • Compiling patient information from referral sources.
  • Entering referrals into Homecare Homebase.
  • Coordinating care with nursing supervisors and the interdisciplinary team.
  • Providing effective communication to patients, staff, and other healthcare professionals.
  • Serving as the first line of customer service.
  • Maintaining accurate records on referral sources and patient status.
  • Performing reverse marketing to reinforce messaging.
  • Meeting assigned key performance indicators (KPIs).
  • Coordinating with other departments for insurance verification and authorization.
  • Participating in department in-service trainings and QAPI program.
