- Job Title: Hospice Team Assistant
- Job Summary: The Hospice Team Assistant plays an essential role in supporting our hospice care team by ensuring seamless coordination and communication.
Job Qualifications
- Education: High school diploma required.
- Licensure: Current Driver’s License.
- Experience: Office experience required; prior experience in hospice or healthcare settings preferred.
- Skills: Proven ability to multitask, prioritize, and adapt to changing responsibilities in a fast-paced environment. High attention to detail with strong organizational and follow-up skills. Proficient in Microsoft Office Suite (Word, Excel, Outlook, Teams) with the ability to quickly learn and navigate hospice-specific software systems. Skilled in operating multi-line phone systems and managing high call volumes with professionalism and accuracy. Excellent written and verbal communication skills and a commitment to providing compassionate, efficient support to the clinical team, patients, and families.
- Transportation: Reliable transportation and valid auto liability insurance.
Environmental and working Conditions
- Works in a routine office environment. Noise level may be moderately high, the ability to work a flexible schedule and extended hours. The ability to travel locally and some exposure to inclement weather. Possible exposure to toxic materials, toner, etc. Requires the ability to handle stressful situations in a calm manner.
Essential Functions
- Manage the supply room, including ordering, receiving, and maintaining inventory, and retrieving supplies for staff and patients as needed.
- Facilitate clear and timely communication between healthcare professionals, patients, and families to ensure coordinated care.
- Maintain accurate and organized patient records in accordance with hospice standards and policies.
- Provide comprehensive administrative support to the hospice care team, including scheduling, documentation, and communication needs.
- Coordinate and prepare for meetings, including assisting with the organization and documentation of IDG (Interdisciplinary Group) meetings.
- Answer multi-line phone systems and provide exceptional customer service to families, patients, healthcare providers, and internal team members.
- Process and track DME (Durable Medical Equipment) orders to ensure timely delivery and proper documentation.
- Assist the Clinical Director and RN Team Leader with non-clinical audits by reviewing charts to ensure documentation completeness and accuracy in required notes and forms.
- Perform other duties as assigned or as needed by the RN Team Leader and/or Clinical Director.
