What You Will Do as a Hospice LPN Field Nurse At Dignity Care Hospice, a Hospice LPN Field Nurse career is more than a job or “a calling” – it’s a chance to leave a legacy of service and love on someone’s life. As a Hospice LPN Field Nurse, you will plan, organize, and direct home care services with an emphasis on community health education and experience. You will build from resources of the community to plan and direct services to meet the needs of patients and families within their homes and communities. Some of your primary responsibilities will include: Completing an initial assessment of patient and family to determine home care needs. Providing a complete physical assessment and history of current and previous illness(es) Supervising and directing the Licensed Practical Nurse (LPN) and/or the Home Health Aide in implementing the patient care plan Developing patient education plans, as needed, and directing the LPN in implementing said plans Creating and modifying the Home Health Aide care plan and supervising them via patient feedback and onsite interactions Evaluating and revising plan of care as patient status and needs change Using health assessment data to determine nursing diagnosis Developing a plan of care, which establishes goals based on nursing diagnosis and incorporating therapeutic, preventive, and rehabilitative action.
