  • What We’re Looking For
  •  
  • Trillium Health Resources has a career opening for a High Fidelity Wraparound (HFW) Liaison to join our team! The High Fidelity Wraparound (HFW) Liaison is responsible for directly supporting, monitoring, and collaborating with state and other partners in HFW expansion efforts using a state existing fidelity monitoring system through the implementation of expansion funds. This position will be the point of contact for recruiting, identifying, developing, tracking, monitoring, and evaluating HFW in our region, as well as program outcomes and requirements, and developing and maintaining relationships with identified HFW providers, state partners, and community stakeholders. This position will work in collaboration with HFW partners to increase community awareness of HFW and expansion of Trillium’s HFW provider network. This position requires some travel.

On a typical day, you might

  • * Monitor and manage recruitment and identification of new and existing HFW teams in counties with expansion needs.
  •  
  •  
  • * Collaborate with HFW partners to increase awareness and understanding of high-fidelity wraparound services, fostering collaboration and engagement. 
  • * Monitor implementation of provider HFW contracts and allocation funds.
  • * Develop and refine payment processes to ensure efficient utilization of resources.
  • * Serve as the liaison between providers and the Finance Department to resolve issues.

  • Qualifications
  •  

Required

  • * Master’s degree in Human Services field and a minimum of two (2) years of experience with behavioral/mental health, substance use, and/or intellectual/developmental disability populations.
  • * Must have a valid driver’s license
  • * Must reside within North Carolina. 
  • * Must be able to travel within catchment as required.
  •  

Preferred

  • * Experience with High Fidelity Wraparound (HFW). 
  • * Experience with evidence-based practices (EBPs) proven effective for youth with or at risk of a Serious Emotional Disturbance (SED).
