Position Summary

The Clinical Documentation Reviewer is responsible for reviewing home care documentation for accuracy, completeness, and compliance with regulatory requirements. The ideal candidate will have hands-on experience with RN Pad and HHA Exchange, as well as strong knowledge of home care assessments and care coordination. This position does not require an RN license, but prior clinical documentation review experience in the home care industry is essential.

Key Responsibilities

  • Review, audit, and validate clinical documentation to ensure accuracy and compliance with agency standards, state, and federal regulations.
  • Verify completion and accuracy of assessments, care plans, and visit notes.
  • Utilize RN Pad and HHA Exchange systems to track, review, and update documentation.
  • Communicate with clinical staff and supervisors regarding missing, incomplete, or incorrect documentation.
  • Provide feedback and support to ensure proper documentation practices.
  • Assist in quality assurance initiatives and prepare reports as needed.
  • Maintain confidentiality of all patient records in accordance with HIPAA and agency policies.

Qualifications

  • Minimum of 2 years’ experience in home care documentation review, quality assurance, or clinical coordination.
  • Proficiency with RN Pad and HHA Exchange systems required.
  • Strong knowledge of home care assessments, care planning, and regulatory compliance.
  • Exceptional attention to detail and organizational skills.
  • Ability to work independently and meet deadlines.
  • Strong written and verbal communication skills.
  • High school diploma or equivalent required; Associate or Bachelor’s degree in Health Care Administration or related field preferred (not required).

Job Types: Full-time, Contract

Pay: $19.00 per hour

Expected hours: No more than 40 per week
