  • About our Centralized Follow-Up Advanced Practice Clinician position
  • Under the supervision of the Clinician Manager for Centralized Follow-Up [CFU] and the Chief Medical Officer, provides post-visit care to patients seeking reproductive health care in Health Centers in all affiliate states. Supports patients and staff by performing, with accuracy and timeliness, a variety of CFU tasks and incorporates those functions into the overall operations of the affiliate.
  • What will you be doing!
  • • Patient Care: Provides direct and indirect patient medical care that is evidence-based and aligns with affiliate protocols. Performing tasks within the relevant scope of practice, including but not limited to triage, case management, follow-up on referrals and abnormal laboratory results, patient education, medical records review, and chart review.
  • • Maintaining Knowledge: Maintains knowledge of the affiliate Medical Standards and Guidelines. Maintains ongoing competency and continuing education required for licensure and current knowledge needed to perform work tasks.
  • • Proficiency in Electronic Health Record [EHR] System: Exhibits ongoing proficiency in electronic practice management systems/EHR (Epic), health center policies, procedures, and practices. Provides medical care in the electronic medical record environment. Documents in medical record and practice management systems. Schedules appointments as needed.
  • • Teamwork/Collaboration: Works within a team environment to render highly professional and technical medical care to patients. Assists Clinical Services leadership in developing policies and procedures that promote teamwork, quality medical care, and enhanced customer service.
  • • Professionalism: Acts as a role model and resource in the development of follow-up systems. Managing communication with patients, colleagues, and persons external to the affiliate in a professional, systematic, and organized manner. Provides quality and efficient telecommunication or electronic customer service to internal and external customers
  • • Quality: Assists in quality assurance tasks via auditing lab results, referrals, and follow-up plans to ensure completion of follow-up and adherence to established protocols based on affiliate Medical Standards and Guidelines. Completes quality assurance monitoring tools as specified for this department.
  • • Job Knowledge, Skills, and Abilities: Must be willing to orient in surgical/family planning environment to develop in-depth knowledge of health center flow and functionality.

Key Responsibilities

  • The APC is assigned daily to a Centralized Follow-Up role and assumes the duties and responsibilities for that role for that shift; may offer support to other roles as time permits. The following are the major responsibilities across all Centralized Follow-Up Roles that may be needed as part of the CFU APC's responsibilities.
  • • Medical Skills: Provides professional and comprehensive medical assessment, evaluation, education, and advice to patients within affiliate protocols (see “Other” below). Delegate work to Registered Nurses [RNs] within the scope of that licensure. Assists and serves as consult resources to CFU/Triage RNs for aspects of patient care that are beyond the RN scope of practice.
  • • Follow-Up: Manages follow-up activities for abnormal results or physical findings. Activities may include maintenance of follow-up systems, notification of patients of results, scheduling of appointments, coordination of referrals to appropriate health care resources, and documentation of compliance with recommended medical regimens.
  • • Patient Education: Provides patient education regarding topics within the scope of care provided by the affiliate, including but not limited to contraception, sexually transmitted diseases (including HIV) prevention and treatments, health promotion activities, pregnancy options, gender affirming hormone care, or abortion services. Provides appropriate and medically accurate patient education in support of all services offered by the affiliate.

• Communication: Checks and responds to messages and communications via printer, fax, email, and EHR tasks. Triages patient calls appropriately based on affiliate protocols. Respond to patient calls by the end of the business day. Respond to MyChart messages within 2 business days. Expedites urgent/emergent calls as needed. Receive, manage, and direct calls in accordance with call management protocols, including but not limited to

  • • Routes calls to appropriate department for continued assistance, as necessary.
  • • Communicate requests and respond accurately using the appropriate procedures for contacting health centers and/or health center staff.
  • • Maintain thorough knowledge and understanding of health operations to answer questions and direct calls correctly and appropriately.
  • • Documentation: Documents interventions and education in the EHR of established patients using standing medical guidelines, abbreviations, and EHR documentation standards.
  • • Training: In conjunction with/at the request of the CFU Clinician Manager or clinician, assist the Education/Training Department in identifying clinical procedures and CFU processes in need of review for existing staff or new employees. Assists with training, precepting, and mentoring of new or existing staff. Acts as a medical resource for affiliate staff.

Minimum Qualifications

EDUCATION

  • • MA/MS degree in related field preferred.
  • • ARNP or PA-c license and specialty certification required.

EXPERIENCE

  • • Minimum two years' experience as an ARNP or PA-c required.
  • • Women’s Health or Reproductive Health experience preferred.
  • • Bilingual English/Spanish skills preferred.

LICENSE

  • • Current RN and ARNP licenses in Washington State.
  • • Must be willing to obtain RN, ARNP, and DEA licenses in other affiliate states (Alaska, Hawai’i, Idaho, Indiana, and Kentucky)
  • • Must reside in Washington State.

CERTIFICATION

  • • Current CPR for Health Care Providers certification is required if employee will be physically present in a health center at any time during their employment.

Salary Range

  • $34.07 - $74.44 hourly

Position Type

  • • Fully remote position – must have HIPAA-compliant remote set up and capacity to connect reliably to the internet.
