  • Arms Acres is a licensed facility providing inpatient and outpatient treatment for those suffering from addiction to alcohol or drugs.
  • Sign-on bonus eligible
  • To provide comprehensive clinical case management services that include: assessments; individual, group and family treatment; treatment plan coordination; referent relations; discharge planning; and intervention with client support systems, including social service programs and managed care.
  • ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties as required.
  • Provides/ensures comprehensive evaluation of patient's functioning as defined by facility policy and procedures.
  • Coordinates the development and implementation of the multidisciplinary treatment plan; coordinates treatment plan review; performs these activities within established time frames. Documents patient progress and own interventions in case record. Need to remain within time frames of regulatory requirements.
  • Provides individual, group and family treatment (psycho-education, life skills) as identified in patient treatment plans and requested by program director.
  • Performs utilization review, provides progress reports and develop aftercare plans in conjunction with referring persons/agencies, EAPs and managed care/insurance entities.
  • Attend and/or facilitate Family program.
  • Responds to emergency situations in the facility by following appropriate procedures for ensuring patient safety during emergency procedures.
  • Maintains professional competence in discipline and treatment specialty areas through continuing education and supervision.
  • Monday through Friday Monday, Tuesday and Friday -11am-7:30pm Wednesday and Thursday 9am-5:30pm

  • Required qualifications are CASAC-T, CASAC certification or LMSW, LMHC
