About the role The permanent part-time (4 days/week) role of the Ambulatory Service Coordinator is to work within the Mission and Mission in Action of HammondCare to lead and develop a high quality, efficient and effective ambulatory care rehabilitation service. The Ambulatory Service Coordinator will also provide an effective liaison role between internal and external stakeholders, including patients and their carers, facilitating understanding and relationship-based care. Key Focus Areas To lead the planning, management, and delivery of ambulatory care rehabilitation services including allied health, medical and administration team members in a manner that supports the provision of effective and efficient rehabilitation services to patients and their carers. Participate as a member of the Healthcare and Hospitals team in the application of the organisation’s Mission and values to the pursuit of its objectives in a patient centred, continuous quality improvement work environment. Advocate for Healthcare and Hospitals ambulatory rehabilitation services by openly promoting service capability and capacity. Proactively and creatively cultivate relationships and develop strategic opportunities to grow ambulatory services. HammondCare is a NSW Health-affiliated organisation all NSW Health transferrable entitlements will be recognised. Essential criteria A tertiary degree in allied health (or equivalent) and current registration with Australian Health Practitioner Regulation Agency. Demonstrated significant practical experience in a rehabilitation setting, inclusive of both ambulatory care and inpatient services. Demonstrated management competencies including service planning, resource management and leading teams. Demonstrated ability to create, implement, report on and monitor service key performance indicators (KPIs). Demonstrated effective communication and interpersonal skills (written and verbal). Strong administrative and computer skills. Demonstrated analytical skills to identify risk and make recommendations for change. Demonstrated time management and organisational skills to effectively plan and prioritise work Willingness to travel and work between HammondCare’s sites and services as required. The ability to develop relationships with key stakeholders, preferably within healthcare funding such as private health insurers is desirable All applications must address the selection criteria (under headings) in a brief cover letter in addition to the resume.
